The inspection visit took place between August 26 and September 15.
In the urgent and emergency care and medicine services the inspectors found limited training in key skills and staff did not always know how to protect patients from abuse and manage safety well. Risk to patients was not always assessed well and acted upon and record keeping was not clear, up-to-date or easily available to staff. There was not always enough staff to care for patients and keep them safe.
Access to the urgent and emergency care and medicine services was not always possible when needed and there were often long waits for treatment.
