Inspection took place May 25-27 and June 2-29, 2021.

This was an unannounced inspection, following the rating of two mental health services provided by the Trust as inadequate and four others as well as a community health service as requires improvement at the previous inspection. Overall, the Trust was rated as inadequate for well-led.
