The inspection visit took place on December 2, 2020. This was an unannounced focused inspection in response to concerns raised through enquiries and serious incident reporting about: medical care services in relation to discharge processes and communication; venous thromboembolism assessment and management; incident reporting and sharing of learning including Never Events; support, care and treatment for patients with learning difficulties; staffing; poor culture and emotional control; infection prevention and control; and allegations of bullying. Key questions were only re-rated where a breach of regulation was identified.
