The inspection visit took place between September 28 and October 4, 2021.

This was an unannounced, focused inspection of maternity and surgery following the receipt of information of concern about the safety and quality of the service. The information came from several sources, including staff whistleblowing, patient complaints and other regulatory bodies. The inspection focused on the safety and well-led key questions, and both services were rated as inadequate.
