The inspection visit took place between June 14 and August 5, 2021.
This was an unannounced inspection of forensic inpatient wards as the CQC was made aware of issues including unsafe staffing numbers and poor culture within the service. A separate, short notice (24 hours) inspection of some community mental health services was also carried out following the receipt of information giving cause for concern over the safety and quality of these services.
Following the inspection a warning notice was issued under Section 29A of the Health and Social Care Act 2008 identifying specific areas that the Trust must improve, with a date for overall compliance of March 1, 2022.
